Community Leadership Award

Congratulations to our 2024 winner, Phoebe Worth Medical Center!

The GHA Community Leadership Award was established in 1997 to recognize a GHA-member hospital that has made exemplary contributions to the health and well-being of the people of Georgia through leadership in the area of community health improvement. Any GHA-member hospital may make a nomination. Joint nominations are encouraged in cases of collaborative efforts involving more than one GHA-member organization.

Applicants are asked to meet the following criteria for excellence in serving Georgia's health care community:

  • Outstanding Community-Based Contribution: The hospital shall have made, or be making, a truly outstanding contribution through an initiative to improve the health status of individuals, targeted groups, communities or the state. (750 words or fewer.)
  • Community Connections: The hospital has developed an innovative, collaborative approach to meeting an identified community need(s) to advance health status and/or reduce health disparities. (750 words or fewer.)
  • Results: As a result of the hospital's efforts, there is concrete evidence of heightened community involvement in addressing local and health care needs. The efforts have also instilled within the community a greater sense of affiliation with the hospital and a better understanding of the role it plays in the vitality of the community. (750 words or fewer.)

Nominations

Any GHA member may submit a nomination. A nomination may be submitted in narrative form summarizing the purpose and accomplishments. To be considered for review, nominations should not exceed 10 double-spaced pages. Note: A hospital may nominate its own programs.
