- Josh Nahum Special Award for Achievement in Infection Prevention and Control
*Circle of Excellence Award (no application required) is awarded to a hospital that wins an award in the current year as well as three or more PHA Patient Safety and Quality awards within the previous five years.
Patient Safety Awareness Week logo competition
Now is your chance to create an original theme and logo that encourage staff to engage with patients and families to improve safety, communication, and care coordination. To submit a design for the Patient Safety Awareness Week Logo Contest, please email your design and completed attestation to Shondia Evans. The winning logo will be unveiled and the awards will be presented at the GHA Patient Safety and Quality Summit in January 2026. Deadline for awards and logo submissions is Friday, Sept. 19, at 5 p.m.
