PHA Reportable Events

Reportable Events Task Force is composed of risk managers, pharmacists, QI/PI staff and nurses. The ad hoc task force was created as a subset of both the Safe Medication Use and PSI-ME Sub-Committees to better define sentinel events. The task force will also develop the report tool and methodology by which HCOs will collect sentinel event information from and report it to the PHA Accountability and Health Safety Program.
