PHA is based on bringing together the individual, the provider, and the community to improve the quality and safety of health and health care. Please use the resources to help guide you in your efforts to live healthy and become an active member of your health care team.

Who
A collaborative group of individuals from around the State of Georgia interested in improving the outcomes to improve the health of Georgia.
What:

Collaborative efforts and activities explore the causes of health care practice variations and support wellness and prevention programs in the following areas (click each area for statistics on each initiative):

How:

Hospitals, health systems and other providers serve as a catalyst and play leadership roles, to facilitate the identification of their community needs and to help individuals achieve their optimal health. This is achieved through:

  • Actively seeking out community members to serve as the patient and community advocates.
    Provide user-friendly reports and forums to engage community members in addressing needs.
  • Support education efforts and materials to make patients fully informed of risk factors and the treatment options.
  • The group supports improvement in community health by encouraging use of best practices, safety, care management programs, preventative medicine, health promotion, and early intervention.

Goals

The goals are based upon the conviction that the healthcare field should work together with others to achieve the vision of healthy communities. Specific goals of the PHA program include:

  • Strengthen ties and intensify collaborative efforts with other organizations with common values.
  • Advocate and assist in providing leadership for the ongoing improvement in the delivery of health services that is demonstrated through measurements of access, patients' perception of care, resource use, and quality
  • Provide a forum to discuss and advocate for public policy to create an environment conductive to the development and operation of integrated delivery that provides coordinated care and to expand health care coverage and access to these health care services.
