Welcome to the PHA Core Measures website!

The purpose of this website is to provide “one stop shopping” for people who are searching for more information about the The CMS/Joint Commission Core Measures and how to meet the goals.

Ready to get started? On the left side of the page (in the red area) are the major Core Measure categories. If you click on any of the Core Measure buttons, you are taken to a page of information specifically related to that Core Measure. The top "red" button is for all Core Measures and provides more general resources.

There are buttons at the bottom of each page on this website if you need more information. For a link to the various Core Measures, please use the links at the bottom of the page for more information from Joint Commission or CMS (MEDQIC). Our Power Hours are designed to be a free resource to help you do your job. They are held monthly, so be sure to register. They tend to fill up quickly. The PHA button will take you to the home page for the Partnership for Health and Accountability. If you need to contact someone at PHA, you can click on the "Contact Us" button at the bottom and a new window will open using your designated email program.

PHA also has an Immunization Toolkit for information specifically related to pneumonia and influenza.

If you are interested in Benchmarking information, use the button at the bottom to go to the PHA website and click on the box for Data Submission. Make a selection from there for the information that you need. Please note that you will need a PHA login and password to access this part of the PHA website.
