PHA Committees - Accountability & Health Safety (AHS)
Purpose
To promote evidence-based best practices to enhance patient safety, improve care, reduce risk and prevent errors through creating a blame-free system and neutral non-punitive forum for sharing, studying, and learning from medical variances, including near misses.
Sub-Committees and Task Forces
- Peer Review Panel is composed of MDs, nurses, pharmacists, risk managers, QI/PI staff. The panel will request, receive and maintain peer review data on behalf of A&HS. The panel is responsible for reviewing events, analyzing de-identified data, and identifying trends for education or medical alerts as shared with A&HS by participating healthcare organizations. To direct their work, the Peer Review Panel will also maintain regularly reviewed and updated guidelines.

PHA Committees - Advisory Council
The role of the PHA Advisory Council is to provide advice, develop consensus and make recommendations on major issues and communicate that information to their constituent groups and others that affect health policy throughout the state. Membership will be changed as needed to provide the leadership vision and skills to guide the evolution of PHA. Representatives are selected, based on their leadership positions in the hospital, business, provider, public health and community sectors.
