Be Active in Your Healthcare
Decisions - Take part in decisions about your healthcare. Doctor - Choose a doctor who is experienced in the type of care you need.
Health History - Write down your health history, current problems and any questions, and bring it with you to show your doctor or nurse. Medicines - Write down and keep track of the medicines you take and take this information to the doctor with you.
Instructions - Go over instructions and have your doctor or nurse write them down so that you may read them at a later time. Friend/Family - Have a friend or family member with you to help you understand what your doctor says.
Learn About Your Condition - and treatments by asking your doctor, pharmacist, nurse, and by using other reliable sources. Healthcare Giver - know the role each doctor plays in your care--this is important if you have many health problems or are in a hospital.
Ask questions - Ask questions that help you understand your care
